    WHAT WE DO:
      A point in time which describes the release or loss of control over a hazard. The undesired system state. A main event describes the point at which we no longer have satisfactory control over the hazard. Normally it is what we consider an unsafe state that is not yet an accident.
        Therefore, major events, while not disasters in themselves, have the potential to become one if nothing is done to control them and could create considerable losses.
            The top event represents the moment when there is loss of control over the hazard and the potential for harm is realized, for example, when containment of a hazardous material is lost. The term derives from fault tree analysis where the undesired event lies at the top of the tree that is used to show its causes.
