      For incidents in the precursor section of the new triangle, action plans must carefully identify and address the behaviors, analyzing the other elements of the process represented by the different levels of the new triangle that change the way to prevent serious and fatal injuries. It’s unlikely you will find an organization that doesn’t want to prevent serious injuries or fatalities (SIF) at their work site(s).

    WHAT WE DO:
      The traditional Heinrich's triangle can be modified to include the form and context of workers' activities as precursors to serious and fatal incidents. A precursor to serious and fatal incidents is defined as a high-risk situation in which management controls are absent, ineffective, or not followed, and which will result in serious injury or fatality, if allowed to continue.
